Ce ph\u00e9nom\u00e8ne contribue \u00e0 la polym\u00e9dication et \u00e0 l\u2019augmentation des d\u00e9penses de soins. Quelles sont les cascades m\u00e9dicamenteuses fr\u00e9quentes et comment les reconna\u00eetre et les pr\u00e9venir&nbsp;?<\/strong><\/span><\/p>\n<div class='summary'>\n<div class='summary-article-content'>\n<p><span style='color:black;'><span style='font-family:;' lang='FR'><strong>Messages cl\u00e9s<\/strong><\/span><\/span><\/p>\n<ul>\n<li><span style='font-family:;' lang='FR'>Un effet ind\u00e9sirable provoqu\u00e9 par un m\u00e9dicament n\u2019est pas toujours reconnu et peut \u00eatre interpr\u00e9t\u00e9 comme une nouvelle pathologie. Une <strong>cascade m\u00e9dicamenteuse<\/strong> se produit lorsque l\u2019effet ind\u00e9sirable d\u2019un m\u00e9dicament (\u00ab&nbsp;m\u00e9dicament index&nbsp;\u00bb) entra\u00eene la prescription d\u2019un m\u00e9dicament suppl\u00e9mentaire (\u00ab&nbsp;m\u00e9dicament marqueur&nbsp;\u00bb). <\/span><span style='color:black;'><span style='font-family:;' lang='FR'>Les cascades associ\u00e9es \u00e0 une prescription potentiellement inappropri\u00e9e sont d\u00e9nomm\u00e9es \u00ab&nbsp;<strong>cascades m\u00e9dicamenteuses probl\u00e9matiques<\/strong>&nbsp;\u00bb.<\/span><\/span><\/li>\n<li><span style='font-family:;' lang='FR'>Trois p<\/span><span style='color:black;'><span style='font-family:;' lang='FR'>ublications ont d\u00e9fini des cascades m\u00e9dicamenteuses probl\u00e9matiques&#x002C; que nous reprenons aux<\/span><span style='font-family:;' lang='NL'>&nbsp;<\/span><\/span><a href='#tabel%201'><span style='font-family:;' lang='FR'>Tableau&nbsp;1<\/span><\/a><span style='color:black;'><span style='font-family:;' lang='FR'>&#x002C;&nbsp;<\/span><\/span><a href='#tabel%202'><span style='font-family:;' lang='FR'>Tableau&nbsp;2<\/span><\/a><span style='color:black;'><span style='font-family:;' lang='FR'> et&nbsp;<\/span><\/span><a href='#tabel%203'><span style='font-family:;' lang='FR'>Tableau&nbsp;3<\/span><\/a><span style='color:black;'><span style='font-family:;' lang='FR'>. Les cascades mentionn\u00e9es dans deux ou trois des trois tableaux sont les suivantes&nbsp;:<\/span><\/span>\n<ul>\n<li><span style='color:black;'><span style='font-family:;' lang='FR'>antagoniste du calcium <strong>&gt;<\/strong> \u0153d\u00e8me p\u00e9riph\u00e9rique <strong>&gt;<\/strong> diur\u00e9tique<\/span><\/span><\/li>\n<li><span style='font-family:;' lang='FR'>benzodiaz\u00e9pine <strong>&gt;<\/strong> <\/span><span style='color:black;'><span style='font-family:;' lang='FR'>troubles de la m\u00e9moire et de la concentration <strong>&gt;<\/strong> m\u00e9dicament anti-Alzheimer<\/span><\/span><\/li>\n<li><span style='color:black;'><span style='font-family:;' lang='FR'>antipsychotique <strong>&gt;<\/strong> sympt\u00f4mes extrapyramidaux <strong>&gt;<\/strong> antiparkinsonien<\/span><\/span><\/li>\n<li><span style='color:black;'><span style='font-family:;' lang='FR'>AINS <strong>&gt;<\/strong> \u00e9l\u00e9vation de la tension art\u00e9rielle <strong>&gt;<\/strong> antihypertenseur<\/span><\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style='color:black;'><span style='font-family:;' lang='FR'>Pour \u00e9viter les cascades m\u00e9dicamenteuses&#x002C; il est important d\u2019envisager que tout nouveau sympt\u00f4me puisse \u00eatre caus\u00e9 par un m\u00e9dicament&#x002C; en particulier peu de temps apr\u00e8s une instauration ou une modification de dose (voir aussi <\/span><\/span><a href='#tabel%204'><span style='color:black;'><span style='font-family:;' lang='FR'>Tableau 4<\/span><\/span><\/a><a href='#Tabel_5'><span style='color:black;'><span style='font-family:;' lang='FR'>).<\/span><\/span><\/a><\/li>\n<li><span style='color:black;'><span style='font-family:;' lang='FR'><strong>Commentaire du CBIP&nbsp;:&nbsp;<\/strong>La polym\u00e9dication est un probl\u00e8me croissant chez les personnes \u00e2g\u00e9es et vuln\u00e9rables et les cascades m\u00e9dicamenteuses y contribuent en (petite) partie. La reconnaissance de cascades m\u00e9dicamenteuses probl\u00e9matiques et les lignes directrices visant leur pr\u00e9vention et leur r\u00e9solution contribuent \u00e0 s\u00e9curiser l\u2019usage des m\u00e9dicaments. La cause est souvent une interpr\u00e9tation erron\u00e9e de l\u2019effet ind\u00e9sirable comme \u00e9tant une nouvelle condition m\u00e9dicale.<\/span><\/p>\n<p><span style='font-family:;' lang='FR'>Il arrive parfois qu\u2019un nouveau m\u00e9dicament soit d\u00e9lib\u00e9r\u00e9ment instaur\u00e9 pour pr\u00e9venir ou traiter les effets ind\u00e9sirables d\u2019un m\u00e9dicament&nbsp;: par exemple&#x002C; instauration d\u2019un laxatif en cas d\u2019utilisation chronique d\u2019un opio\u00efde&#x002C; ou d\u2019un IPP chez les patients \u00e0 risque pendant un traitement par AINS.<\/span><\/p>\n<p><span style='font-family:;' lang='FR'>Toutefois&#x002C; il n\u2019est parfois pas souhaitable de combiner le m\u00e9dicament index et le m\u00e9dicament marqueur et il peut y avoir des cons\u00e9quences n\u00e9gatives&nbsp;: ce sont les <strong>cascades m\u00e9dicamenteuses&nbsp;probl\u00e9matiques<\/strong>&#x002C; qui sont au c\u0153ur de cet article de Folia.&nbsp;Un exemple&nbsp;:<\/span><\/p>\n<p style='margin-left:80px;'><span lang='FR'>IECA (m\u00e9dicament index) <strong>&gt;<\/strong> toux (effet ind\u00e9sirable) <strong>&gt;<\/strong> cod\u00e9ine (m\u00e9dicament marqueur).<\/span><br \/><span style='font-family:;' lang='FR'>Cette cascade est jug\u00e9e probl\u00e9matique&#x002C; car il existe des alternatives \u00e0 l\u2019IECA et que la cod\u00e9ine peut elle-m\u00eame provoquer des effets ind\u00e9sirables alors qu\u2019elle n\u2019est pas efficace sur une toux caus\u00e9e par un IECA.<\/span><\/p>\n<p><span style='font-family:;' lang='FR'>Les cascades m\u00e9dicamenteuses probl\u00e9matiques participent \u00e0 la polym\u00e9dication&#x002C; tout en ajoutant possiblement de nouveaux effets ind\u00e9sirables provoqu\u00e9s par le m\u00e9dicament marqueur. Elles peuvent aussi donner lieu \u00e0 une qualit\u00e9 de vie r\u00e9duite&#x002C; \u00e0 un nombre suppl\u00e9mentaire de renvois vers d\u2019autres professionnels de la sant\u00e9 et \u00e0 une augmentation des frais de sant\u00e9&#x002C; entre autres.<\/span><\/p>\n<p><span style='font-family:;' lang='FR'>Les recommandations en mati\u00e8re de reconnaissance et de pr\u00e9vention des cascades m\u00e9dicamenteuses probl\u00e9matiques fr\u00e9quentes peuvent \u00eatre pr\u00e9cieuses pour les professionnels de la sant\u00e9.<\/span><\/p>\n<h2><a class='bcfi-anchor' id='2' name='2'>&nbsp;<\/a><span style='font-family:;' lang='FR'>9&nbsp;cascades m\u00e9dicamenteuses cliniquement significatives chez les patients \u00e2g\u00e9s<\/span><\/h2>\n<p><span style='font-family:;' lang='FR'>Un groupe d\u2019experts internationaux a s\u00e9lectionn\u00e9 &#8211; dans la publication de<strong> McCarthy et al (2022)<\/strong><\/span><span style='color:#00B050;'><span style='font-family:;' lang='FR'><strong><sup>1<\/sup><\/strong><\/span><\/span><span style='font-family:;' lang='FR'> &#8211; <strong>9&nbsp;cascades m\u00e9dicamenteuses cliniquement significatives chez les patients \u00e2g\u00e9s qui sont associ\u00e9es \u00e0 une prescription potentiellement inappropri\u00e9e&nbsp;<\/strong>(p<\/span><span style='font-family:;' lang='FR-BE'>rovenant de 139 cascades d\u00e9crites dans l\u2019article<\/span><span style='font-family:;' lang='FR'>). Le groupe d\u2019experts se composait de g\u00e9riatres&#x002C; de g\u00e9n\u00e9ralistes&#x002C; de pharmaciens et d\u2019infirmiers venus du Canada&#x002C; des \u00c9tats-Unis&#x002C; d\u2019Isra\u00ebl&#x002C; de Belgique&#x002C; d\u2019Irlande et d\u2019Italie. Les auteurs ont retenu 19&nbsp;cascades m\u00e9dicamenteuses sur les 94 analys\u00e9es. Le <\/span><a href='#tabel%202'><span style='font-family:;' lang='FR'><strong>Tableau&nbsp;2<\/strong><\/span><\/a><span style='font-family:;' lang='FR'> \u00e9num\u00e8re ces 19&nbsp;cascades.<\/span><\/p>\n<div class='detailed-content'>\n<p><span style='font-family:;' lang='FR'>Les \u00ab preuves robustes \u00bb sont d\u00e9finies comme suit&nbsp;: (1) un minimum de deux \u00e9tudes observationnelles de bonne qualit\u00e9 ont \u00e9tabli une association significative entre le m\u00e9dicament index et le m\u00e9dicament marqueur et aucune \u00e9tude n\u2019a identifi\u00e9 d\u2019effet oppos\u00e9&#x002C; (2) l\u2019effet ind\u00e9sirable est mentionn\u00e9 dans la notice am\u00e9ricaine&#x002C; et (3) il existe un m\u00e9canisme prouv\u00e9 expliquant que l\u2019effet ind\u00e9sirable est provoqu\u00e9 par le m\u00e9dicament index.<\/span><\/p>\n<\/div>\n<p><span style='font-family:;' lang='FR'>Les cascades reprises au <strong>Tableau&nbsp;2<\/strong> ne sont pas toujours des cascades m\u00e9dicamenteuses probl\u00e9matiques. Il s\u2019agit par exemple de la cascade \u2018acide ac\u00e9tylsalicylique faiblement dos\u00e9 &gt; dyspepsie &gt; IPP\u2019. [<strong>Note du CBIP&nbsp;<\/strong>: Lors de l\u2019utilisation d\u2019acide ac\u00e9tylsalicylique&#x002C; une protection gastrique peut \u00eatre envisag\u00e9e chez les personnes qui sont \u00e2g\u00e9es de plus de 80&nbsp;ans ou qui pr\u00e9sentent une comorbidit\u00e9 majeure&#x002C; avec des ant\u00e9c\u00e9dents d\u2019ulc\u00e8re gastro-duod\u00e9nal ou d\u2019ulc\u00e8re avec complications (h\u00e9morragie&#x002C; perforation)&#x002C; pour autant que les effets ind\u00e9sirables de la prise d\u2019IPP \u00e0 long terme ne contrebalancent pas les b\u00e9n\u00e9fices gastro-intestinaux escompt\u00e9s (cf. <\/span><a href='https:\/\/www.cbip.be\/fr\/chapters\/3?frag=1859'><span style='font-family:;' lang='FR'>R\u00e9pertoire&nbsp;2.1.1.1.<\/span><\/a><span style='font-family:;' lang='FR'>).]<\/span><\/p>\n<p><span style='font-family:;' lang='FR'>Trois cascades m\u00e9dicamenteuses se retrouvent \u00e9galement parmi les \u20189&nbsp;cascades m\u00e9dicamenteuses cliniquement significatives chez les patients \u00e2g\u00e9s\u2019 du Tableau&nbsp;1 (antagonistes du calcium et \u0153d\u00e8me&#x002C; antipsychotiques et sympt\u00f4mes extrapyramidaux ainsi que benzodiaz\u00e9pines et troubles de la m\u00e9moire et de la concentration).<\/span><\/p>\n<p><a class='bcfi-anchor' id='tabel 2' name='tabel 2'>&nbsp;<\/a><span style='font-family:;' lang='FR'><strong>&nbsp;Tableau&nbsp;2. Une publication \u00e9tait s\u00e9lectionn\u00e9e si elle contenait une analyse statistique identifiant ou confirmant la cascade m\u00e9dicamenteuse et si les effets ind\u00e9sirables du m\u00e9dicament index \u00e9taient confirm\u00e9s dans le R\u00e9sum\u00e9 des Caract\u00e9ristiques du Produit (RCP).<\/span><br \/><span style='font-family:;' lang='FR'>Les combinaisons intentionnelles (p.&nbsp;ex. un laxatif en cas de traitement par un opio\u00efde)&#x002C; les cascades insuffisamment \u00e9tay\u00e9es&#x002C; les rapports de cas et les synth\u00e8ses ont \u00e9t\u00e9 exclus.<\/span><\/p>\n<\/div>\n<p><span style='font-family:;' lang='FR'>Les auteurs ont trouv\u00e9 soit des informations relatives \u00e0 la dose-d\u00e9pendance&#x002C; soit des recommandations pour r\u00e9soudre la cascade pour <\/span><a href='https:\/\/link.springer.com\/article\/10.1007\/s40266-023-01072-y\/tables\/3'><span style='font-family:;' lang='FR'><strong>52&nbsp;cascades m\u00e9dicamenteuses<\/strong><\/span><\/a><span style='font-family:;' lang='FR'>.<\/span><\/p>\n<ul>\n<li><span style='font-family:;' lang='FR'>La <strong>dose-d\u00e9pendance<\/strong> a \u00e9t\u00e9 examin\u00e9e et confirm\u00e9e par les auteurs pour <strong>12&nbsp;cascades<\/strong>&nbsp;: <strong>cf.&nbsp;<\/strong><\/span><a href='#tabel%203'><span style='font-family:;' lang='FR'><strong>Tableau&nbsp;3<\/strong><\/span><\/a><span style='font-family:;' lang='FR'>. Cela signifie donc qu\u2019une r\u00e9duction de la dose du m\u00e9dicament index pourrait contribuer \u00e0 pr\u00e9venir ou \u00e0 inverser la cascade. Les \u00e9tudes diff\u00e9raient toutefois en termes de m\u00e9thode et de d\u00e9finition d\u2019une \u00ab&nbsp;dose \u00e9lev\u00e9e&nbsp;\u00bb. De ce fait&#x002C; il est difficile de formuler des recommandations uniformes.<\/span><\/li>\n<li><span style='font-family:;' lang='FR'>Bon nombre de <strong>recommandations<\/strong> \u00e9taient d\u2019ordre g\u00e9n\u00e9ral&#x002C; comme la diminution de la posologie ou l\u2019arr\u00eat du m\u00e9dicament index&#x002C; sans conseils sp\u00e9cifiques pour la s\u00e9curit\u00e9 de ce processus. Pour 22&nbsp;cascades&#x002C; il \u00e9tait recommand\u00e9 de <strong>relayer le m\u00e9dicament index<\/strong>&#x002C; mais des <strong>alternatives concr\u00e8tes<\/strong> n\u2019\u00e9taient propos\u00e9es que dans 3 des 52&nbsp;cas. la cascade m\u00e9dicamenteuse antagoniste du calcium \u2013 diur\u00e9tique<\/h2>\n<p><span lang='FR'>La cascade m\u00e9dicamenteuse <strong>antagoniste du calcium &gt; \u0153d\u00e8me p\u00e9riph\u00e9rique &gt; diur\u00e9tique<\/strong> \u00e9tait abord\u00e9e dans chacune des \u00e9tudes trait\u00e9es.<\/span><br \/><span lang='FR'>Une <strong>\u00e9tude de cohorte canadienne<\/strong><\/span><span style='color:#00B050;'><span lang='FR'><strong><sup>4<\/sup><\/strong><\/span><\/span><span lang='FR'> a examin\u00e9 la fr\u00e9quence et les cons\u00e9quences de cette cascade m\u00e9dicamenteuse dans un groupe de personnes \u00e2g\u00e9es (&gt;&nbsp;65&nbsp;ans).<\/span><br \/><span style='font-family:;' lang='FR'>L\u2019\u00e9tude a inclus quelque 40&nbsp;000&nbsp;patients chez qui un antagoniste du calcium \u00e9tait instaur\u00e9 pour hypertension. Pour \u00eatre inclus&#x002C; les patients ne devaient pas souffrir d\u2019insuffisance cardiaque&#x002C; ni utiliser de diur\u00e9tiques au d\u00e9but de l\u2019\u00e9tude.<\/span><\/p>\n<p><span lang='FR'>La cascade m\u00e9dicamenteuse \u00e9tait pr\u00e9sente chez environ 1&nbsp;900 des 40&nbsp;000&nbsp;patients \u2013 <strong>pr\u00e8s de 5&nbsp;%<\/strong> donc \u2013 qui se sont vus prescrire un diur\u00e9tique dans les 90&nbsp;jours suivant l\u2019initiation de l\u2019antagoniste du calcium.<\/span><br \/><span style='font-family:;' lang='FR'><strong>Les effets ind\u00e9sirables graves ont \u00e9t\u00e9 plus fr\u00e9quents dans le groupe \u2018cascade m\u00e9dicamenteuse\u2019<\/strong>. Les effets ind\u00e9sirables graves \u00e9taient d\u00e9finis comme suit&nbsp;: renvoi vers un service d\u2019urgences ou hospitalisation dans les 90&nbsp;jours suivant la prescription du diur\u00e9tique. Il y a eu une augmentation de 21&nbsp;% dans le groupe \u2018cascade m\u00e9dicamenteuse\u2019 (donc&#x002C; avec le diur\u00e9tique) par rapport au groupe sans diur\u00e9tique.<\/span><\/p>\n<h2><a class='bcfi-anchor' id='6' name='6'>&nbsp;<\/a>Pr\u00e9venir et r\u00e9soudre les cascades m\u00e9dicamenteuses<\/h2>\n<p><span style='font-family:;' lang='FR'>Le <\/span><a href='https:\/\/www.farmacotherapeutischkompas.nl\/farmacologie\/voorschrijfcascades'><span style='font-family:;' lang='FR'>Farmacotherapeutisch Kompas<\/span><\/a><span style='color:#00B050;'><span style='font-family:;' lang='FR'><strong><sup>5&nbsp;<\/sup><\/strong><\/span><\/span><span style='font-family:;' lang='FR'>\u00e9met des recommandations&#x002C; sur la base de la litt\u00e9rature&#x002C; pour pr\u00e9venir et r\u00e9soudre les cascades m\u00e9dicamenteuses&nbsp;: cf. <\/span><a href='#tabel%204'><span style='font-family:;' lang='FR'><strong>Tableau&nbsp;4<\/strong><\/span><\/a><span style='font-family:;' lang='FR'>.<\/span><\/p>\n<p><span style='font-family:;' lang='FR'><strong>Pour pr\u00e9venir les cascades m\u00e9dicamenteuses&#x002C; le conseil le plus important \u00e0 donner aux prescripteurs est d\u2019envisager&#x002C; \u00e0 chaque nouveau sympt\u00f4me&#x002C; qu\u2019il puisse s\u2019agir d\u2019un effet ind\u00e9sirable.<\/strong> Il est \u00e9galement important d\u2019informer les patients et\/ou leurs proches des effets ind\u00e9sirables \u00e9ventuels&#x002C; Les cascades m\u00e9dicamenteuses y contribuent en (petite) partie.<\/span><\/li>\n<li><span style='font-family:;' lang='FR'>La connaissance et la reconnaissance des cascades m\u00e9dicamenteuses ainsi que les lignes directrices visant leur pr\u00e9vention et leur r\u00e9solution contribuent \u00e0 s\u00e9curiser l\u2019usage des m\u00e9dicaments et \u00e0 am\u00e9liorer les soins aux patients. Pour pr\u00e9venir les cascades m\u00e9dicamenteuses&#x002C; le principal conseil est de <strong>toujours envisager&#x002C;&nbsp;\u00e0 chaque nouveau sympt\u00f4me&#x002C; qu\u2019il puisse s\u2019agir d\u2019un effet ind\u00e9sirable<\/strong> d\u2019un m\u00e9dicament&#x002C; en particulier apr\u00e8s l\u2019instauration ou la modification de dose d\u2019un m\u00e9dicament.<\/span><\/li>\n<li><span style='font-family:;' lang='FR'>Toute contribution \u00e0 l\u2019<strong>analyse rationnelle d\u2019une liste de m\u00e9dicaments<\/strong> est la bienvenue.
